Special inspection notice

All municipal environmental protection bureaus, municipal health and family planning commissions, commissions directly manage medical and health institutions:

Medical waste management is related to public health safety and environmental safety, and we must constantly focus on it. In order to further strengthen the management of medical waste, prevent the spread of diseases, protect the environment, and ensure human health, the Provincial Environmental Protection Department and the Provincial Health and Family Planning Commission decided to jointly carry out a special inspection of medical waste management. The relevant matters are hereby notified as follows:

Inspection object

  •  1. Various medical and health institutions at all levels.
  •  2. Units for centralized disposal of medical waste.

Two, check content

(1) Medical and health institutions

  1. Construction of medical waste management system

  The establishment of a medical waste management system, including the formulation of medical waste disposal work procedures and management requirements (including collection, temporary storage, transportation, handover registration, occupational protection) system conditions, and emergency treatment plans for medical waste loss, leakage, proliferation and accidents .

  2. The establishment of specialized agencies and personnel

  Is there a competent authority and a dedicated person for management, and whether relevant training is carried out regularly.

  3. Flow management of medical waste

  The implementation of the medical waste transfer form and the medical waste registration management system of the medical and health institution, and the medical waste transfer registration record (should include the source, type, weight or quantity of the medical waste, handover time, final destination and the signature of the handler).

  4. Collection of medical waste

  (1) Medical waste collection and transportation procedures, special medical waste transportation vehicles, medical waste temporary storage facilities, and transportation equipment regular disinfection;

  (2) The classification and collection of medical waste: Whether the medical waste generation point is divided into infectious, pathological, damaging, pharmaceutical and chemical medical waste collection; whether the culture medium, specimens, bacteria and virus seed storage solution and other high-risk wastes are disposed of Pre-sterilization; whether the special packaging or container for medical waste is used for collection, with warning signs; whether the packaging or container is damaged or leaking, especially whether the medical waste generated by isolated infectious disease patients or suspected infectious disease patients uses double Layer the packaging and seal it in time. Focus on the management of uncontaminated infusion bottles (bags). It is strictly forbidden to mix needles, disposable infusion sets, infusion tubes and other medical wastes.

  5. Temporary storage of medical waste

  (1) Whether the site is far away from food processing area, medical area, and personnel activity area, separate from domestic garbage, and set up separate channels

  (2) Whether the storage place is locked, managed by a dedicated person and has warning signs

  (3) Storage facilities and equipment should be airtight, walls and floors should be flat and leak-proof; openable windows should be installed with iron fences and screens, and flushing facilities should be provided

  (4) Periodic disinfection and cleaning of temporary storage facilities and equipment for medical waste.

  (5) Whether there is any behavior of discarding medical waste, dumping or stacking medical waste at non-storage locations, or mixing medical waste with other wastes and domestic garbage.

  6. Disposal of medical waste

 (1) Check the disposal contract or agreement signed with the medical waste disposal unit;

 (2) Whether there is any unauthorized disposal of medical waste. Whether the disposal method of the institution that disposes of medical waste on its own because it is not covered by the centralized medical waste disposal institution is correct.

 (3) Check the disposal of grid slag, sedimentation sludge, and septic tank sludge (all belong to medical waste) produced by the sewage treatment system.

 

 (2) Units for centralized disposal of medical waste

  1. Implementation of the hazardous waste business license system

  Check the status of the hazardous waste business license corresponding to the business scope, whether the hazardous waste business license has been applied for in accordance with the law, and whether there is an out-of-scope business. Whether the hazardous waste produced by this unit is handed over to a qualified hazardous waste management unit for disposal.

2.Facility operation and management

  Check whether the disposal process is suitable for the type of medical waste that needs to be disposed of, whether the disposal facility and its supporting pollution prevention facilities are operated, whether the airtightness of the medical waste transfer vehicle meets the requirements, and whether the disinfection treatment is carried out in a timely manner as required.

  3. Storage facilities

  Check whether the storage facilities meet the national technical specifications, whether they are collected and stored separately, whether hazardous wastes and non-hazardous wastes are mixed, whether anti-seepage measures are taken, whether there are discarded medical wastes and dumping and stacking of medical wastes at non-storage locations Or the act of mixing medical waste with other wastes and domestic garbage.

  4. Setting of identification signs

  Check whether the medical waste transfer vehicle, packaging, storage site, disposal site, etc. have standardized identification signs.

  5. Implementation of the transfer order system

  Check the records of the "Medical Waste Transfer Card", the original records of the hazardous waste transfer form, whether the transfer of hazardous waste entrusts a qualified hazardous waste transportation unit, and whether there is any transfer or sale of medical waste.

  6. The implementation of ledger management and business reporting system

  Whether to establish a hazardous waste management record book in accordance with the "Guidelines for Recording and Reporting Operating Status of Hazardous Waste Management Units" (Ministry of Environmental Protection Announcement No. 65, 2009), and regularly report operating activities to the environmental protection department; hazardous waste management plans and emergency plans Development status and filing status, hazardous waste declaration and registration status.

  7. Self-monitoring system implementation

  Whether to monitor pollutant discharge by itself or entrust a qualified unit in accordance with regulations.

  8. Disposal capacity

  (1) Whether the centralized disposal agency covers all counties (cities, districts) within its jurisdiction;

  (2) Whether centralized treatment institutions can cover primary medical institutions (including township health centers, village clinics, community health service institutions, outpatient departments, and clinics);

  (3) The main reason why the capacity of centralized disposal units cannot cover all medical institutions in the jurisdiction.

  3. Organizational form

  The inspection was carried out by the self-inspection organized by the competent departments of environmental protection, health and family planning of each city. Among them, the inspection of medical and health institutions is mainly conducted by the health and family planning authority, with the cooperation of the environmental protection authority; the inspection of the centralized medical waste disposal unit is mainly conducted by the municipal environmental protection authority, with the cooperation of the health and family planning authority.

  On the basis of the self-examination of each city, the Provincial Environmental Protection Department and the Provincial Health and Family Planning Commission jointly organized on-site supervision and random inspection of each city.

  Four, inspection requirements

  1. Medical and health institutions: all types of medical and health institutions at all levels account for a certain proportion.

  2. Centralized medical waste disposal units: All are included in the scope of this inspection, and will be evaluated in accordance with the Ministry of Environmental Protection's "Standardized Management Index System for Hazardous Wastes" (Huanban [2015] No. 99), and will be used as the assessment of the standardized management of hazardous waste in each city in 2017 result.

  Five, time arrangement

 (1) Local inspection: June 2017-August 2017

 (2) Sampling check by Provincial Environmental Protection Department and Provincial Health and Family Planning Commission: September-October 2017

  6. Other requirements

  Before September 15, 2017, the municipal environmental protection bureaus and health and family planning committees will jointly submit the self-inspection report and inspection unit name list (name of the unit to be inspected, list of problems) to the Provincial Environmental Protection Department and the Provincial Health and Family Planning Commission. The main content of the self-inspection report of each city shall include the development of self-inspection organization, the generation of medical waste in the region and the centralized disposal, the existing problems and rectification measures, and the recommendations for the next step. In particular, it is necessary to check whether the centralized disposal agency covers all counties (cities) within the jurisdiction. , Districts) and coverage of primary medical institutions (including township health centers, village clinics, community health service institutions, outpatient departments, and clinics), and explain the main reasons for not covering all medical institutions in the jurisdiction, and propose solutions.

  For violations found during inspections, the municipal environmental protection bureaus and health and family planning commissions shall investigate and deal with relevant regulations in accordance with the "Medical Waste Management Regulations", "Medical Waste Management Measures for Medical and Health Institutions" and "Administrative Punishment Measures for Medical Waste Management".
